Crystal Growers Signup Forms
This form should be used by all current Crystal Growers who wish to apply lime to their fields involved in sugarbeet production. All agreements are currently fillable pdf’s. Please fill in data on computer or mobile device, print, sign, and return. Scan and email in pdf or other document format to byproducts@crystalsugar.com is preferred but postal mail is acceptable. Photos of agreements will not be accepted!

- Lime Removal Agreement – Required, only need one per season is needed from the entity removing lime from landfill. If you have hired a contractor to haul your lime who already has a Removal Agreement on file, this form is NOT necessary.
- Site Suitability for Crystal Growers – One Required for EACH FIELD: Need FSA Farm, Tract, and Field ID and a soil test taken within a calendar year of form submission. If unwilling to provide that data, please use the Non-grower Form.
